הדגימות המועברות למעבדה חייבות להיות מקודדות. המפתח לקידוד יישמר בידי החוקר בארץ. החוקר יצרף לבקשתו מכתב (בנוסח המפורט מטה) מאת המעבדה אליה יועברו הדגימות, לפיו היא מחויבת לקיים את הנחיות הוועדה ותנאיה, וכן לקיים את ההוראות הרלבנטיות בדין הישראלי. כמו כן, במכתב יש להתייחס לגורל הדגימות בתום האנאליזה.

We herewith confirm that we are to receive for analysis and/or storage in our laboratories, coded or unidentified DNA samples, in relation with the following research:

We shall strictly adhere to the processes described in the research protocol, patient information form and informed consent form, and comply with legal & ethical requirements under the applicable Israeli law concerning human medical research, the guidelines of the National Committee for Human Medical Research and the general principles stipulated in the WMA Declaration of Helsinki (1964).
We guarantee that special attention shall be warranted to the protection of participants' privacy and medical confidentiality, through effectively securing all samples and related genetic information.
At the conclusion of the study, the coded or unidentified DNA samples received by the company/laboratory, will be returned to the principal investigator in Israel / destroyed (omit the non-relevant)
